Mean weight change at week 72 was minus 15.0, 19.5 and 20.9 per cent with once-weekly tirzepatide 5, 10 and 15 mg, against minus 3.1 per cent with placebo. The most common adverse events with tirzepatide were gastrointestinal, mostly mild to moderate and mainly during dose escalation; adverse events led to discontinuation in 4.3, 7.1 and 6.2 per cent of the tirzepatide groups and 2.6 per cent with placebo. The trial was supported by Eli Lilly.\u003c\/p\u003e\u003cp\u003eSURPASS-2 (Frías and colleagues, New England Journal of Medicine, 2021) was an open-label, 40-week, phase 3 trial in 1,879 patients with type 2 diabetes comparing tirzepatide 5, 10 or 15 mg with semaglutide 1 mg. Glycated haemoglobin fell by an estimated 2.01, 2.24 and 2.30 percentage points with tirzepatide and 1.86 points with semaglutide. Nausea was reported in 17 to 22 per cent of tirzepatide patients and 18 per cent of semaglutide patients, and serious adverse events in 5 to 7 per cent versus 3 per cent. The trial was funded by Eli Lilly.\u003c\/p\u003e\u003ch3\u003eSide effects, contraindication and warnings in the SmPC\u003c\/h3\u003e\u003cp\u003eTable 1 of the UK SmPC lists as very common (1 in 10 or more in clinical studies) nausea, diarrhoea, vomiting, constipation, abdominal pain, decreased appetite, fatigue and hypoglycaemia when used with a sulphonylurea or insulin. Common reactions (1 in 100 to under 1 in 10) include hypersensitivity reactions, dizziness, hypotension-related events, dyspepsia, abdominal distension, eructation, flatulence, gastro-oesophageal reflux disease, gallstones (cholelithiasis), cholecystitis, hair loss, injection site reactions, increased heart rate and increases in blood calcitonin, lipase and amylase. Footnotes record that several of these frequencies differ between trial populations.
