RAD-140 (Testolone)

RAD-140, also known as testolone and by its development code RAD140, is a non-steroidal selective androgen receptor modulator (SARM) first described by Radius Health in 2011. It is a small oral molecule that binds the androgen receptor with high affinity and was designed to act preferentially in muscle and bone rather than in the prostate. What is RAD-140?

RAD-140 belongs to the SARM class: synthetic, non-steroidal ligands of the androgen receptor (AR), the same nuclear receptor that testosterone and dihydrotestosterone act on. The premise of the class is tissue selectivity: strong receptor activation in skeletal muscle and bone, much weaker agonism in the prostate, seminal vesicles and skin. RAD-140 was reported by its discoverers to be a potent, orally bioavailable AR ligand that showed this pattern in animal models.

Radius Health originally explored RAD-140 as a treatment for muscle wasting and, later, as an oral AR-targeted agent for androgen receptor positive breast cancer. The only human data in the literature come from that oncology programme, a small first-in-human phase 1 study in postmenopausal women with metastatic breast cancer. There are no published controlled trials of RAD-140 in healthy adults, and no data at all on the long-term effects of the compound in people.

RAD-140 has never been approved as a medicine anywhere in the world. Its clinical development did not proceed beyond early phase, and it holds no marketing authorisation from the MHRA, EMA or FDA.

What the published research reports

Discovery and preclinical characterisation (2011). Miller and colleagues at Radius Health described the design and synthesis of RAD140 and its characterisation in several preclinical models of anabolic androgen action, reporting it as a potent, orally bioavailable, non-steroidal SARM. This is a chemistry and animal-model paper; it contains no human data.

Neuroprotection in rat models (2014). Jayaraman and colleagues tested RAD140 in cultured rat hippocampal neurons and in male rats given the excitotoxin kainate. In culture, RAD140 was as effective as testosterone at reducing apoptotic cell death, an effect that depended on MAPK signalling. In gonadectomised adult rats it exhibited peripheral androgen action that largely spared the prostate while protecting hippocampal neurons.

First-in-human phase 1 study (2022). LoRusso and colleagues enrolled 22 heavily pretreated postmenopausal women with ER-positive, HER2-negative metastatic breast cancer in a dose-escalation study. The most frequent treatment-emergent adverse events were elevated AST (59.1 per cent of patients), elevated ALT (45.5 per cent), raised total bilirubin (27.3 per cent), vomiting, dehydration and decreased appetite and weight. Grade 3 or 4 adverse events occurred in 72.7 per cent of patients, including AST and ALT elevations and hypophosphataemia. Clinical benefit rate at 24 weeks was 18.2 per cent and median progression-free survival was 2.3 months.

Liver injury case report (2020). Barbara and colleagues described a 52-year-old man who developed drug-induced liver injury after taking online products containing RAD-140 and RAD-140 with LGD-4033. Liver biopsy showed diffuse centrilobular canalicular cholestasis with ductular reaction and mild lobular inflammation. Liver enzymes returned to normal roughly three months after both products were stopped.

How to compare them

Start with total milligrams per pack: strength per capsule multiplied by capsules per pack. Two 60-capsule packs at 10mg each contain 600mg, so any difference between them is in price, provenance and testing rather than quantity. Divide the pack price by total milligrams to get cost per milligram, the only figure that compares packs of different strengths fairly. Then look at the batch certificate. A pack with an independently issued COA for its lot number is a known quantity; a pack without one is not, whatever the label says. Our article on price per milligram walks through the arithmetic.

Batch testing and certificates of analysis

A certificate of analysis (COA) answers the two questions that matter about a RAD-140 capsule: is the compound what the label says, and how much is present. A genuine batch certificate is issued by an independent laboratory, names the lot number, and reports identity (by HPLC or mass spectrometry against a reference standard) and assayed purity or content.

Every Free Muscle-brand batch is assayed by Janoshik Analytical, an independent laboratory, and the certificate for the batch you receive is sent on request by email. For other brands we show a COA only where the manufacturer has supplied one for the batch we hold. A generic PDF with no batch number, or one dated long before the product was made, is not a batch certificate and tells you nothing about the capsules in your hand. Legal status in the UK

RAD-140 is not a licensed medicine in the United Kingdom. It has never been granted a marketing authorisation by the MHRA, and it is not an authorised food supplement or a permitted novel food ingredient. It cannot legally be sold or labelled for human consumption. Free Muscle sells RAD-140 to adults aged 18 or over for laboratory and research purposes only, and every listing states this. RAD-140 is named on the World Anti-Doping Agency Prohibited List under S1.2, Other Anabolic Agents, and is prohibited at all times. Storage and handling

Keep RAD-140 capsules in the original sealed container in a cool, dry place away from light and heat. Do not decant into unlabelled containers, and record the batch number so that a COA request can be matched to the exact material. Keep out of reach of children and pets, and follow standard laboratory good practice: gloves when handling, no eating or drinking in the work area, and disposal in line with local rules for research chemicals.
